By submitting my photo I,

1. Give consent to the photographic image provided for submission in our Coastal Adaptation Plan invitation for past photos and to Bellarine Bayside, its employees and those acting with its authorisation, displaying my photograph on the Coastal Adaptation Plan Platform.

2. Give consent to my photograph being used within Bellarine Bayside publications, promotional and marketing material, including at events, in videos, catalogues, magazines, newspaper articles, brochures and social media and on Bellarine Bayside's website. I acknowledge that these sites may be viewed and accessed outside Australia in countries which are not subject to privacy laws which provide the same level of protection as Australia.

3. Give consent to my photograph being used within the Coastal Adaptation Plan which feeds into the Coastal and Marine Management Plan.

4. Acknowledge that information is collected by the Bellarine Bayside and its authorised contractors in accordance with its Privacy Policy and the Privacy and Data Protection Act 2014 for the purpose of community consultation and engagement.

5. Confirm that I am 18 or older or otherwise have the permission of my parent or legal guardian to submit this photo to this invitation for photo submissions for the Coastal Adaptation Plan and to Bellarine Bayside.

6. Have permission from any recognisable individuals in the photograph to use their image in this submission.
